Hazel Ford

Hazel V. Ford of Columbus, Age 75, Passed away October 16, 2010 at her Daughter's home in Pataskala Ohio. Born in Martin County Kentucky September 8th 1935 to the late John Herman and Neva Davis. Hazel was a Member of Mt. Sterling Free Will Baptist Church. Preceded in death by husband Charles D. Ford: brothers Johnny, Lee, and Earl Davis. Survived by daughters Connie Rae (Dana) Maddy of Kissimmee Fla., and Regina Lynn (Tom) Carr of Pataskala Ohio; Grandchildren Kevin (Itsa) Maddy, Laura (Joshua) Alch both from Fla., Ryan Carr and Nicholas Carr both from Ohio. Great granddaughter Alyssa Alch of Fla., brothers Russell Davis of Wyoming West Virginia, Leonard (Eloise) Davis of Meally Ky. Kenneth (Barbara) Davis of Columbus; many friends and other relatives. A Special thanks to Jenny and Mount Carmel hospice staff for their support and loving care.
